All Carver HSES students 14 years old and older:
If you need your work permit, please see Ms. Pat Logan in room 111 or Main Office.
Ms. Logan will give you a work permit application that must be signed by your parent/guardian.
When you return your working permit application, you will need to bring with you one of the following documents,
to show as proof of age, your student ID card, birth certificate or passport,

Ms. Logan will then issue you your working permit. (blue card).
